Acupuncture

First office visits are $110 and last 90 minutes.
Return visits are $80 and last 60 minutes.
Children are $80 for the first visits and $60 for return visits.

Cranial Sacral Therapy

60 Minutes-$120

Massage & Pregnancy Massage

60 Minutes-$75
90 Minutes-$105

Maya Abdominal Massage

First Visits are $175 and last 90-120 minutes. This visit includes treatment, self-care instruction handouts and castor oil kit to take home.

Return Maya visits are $90

Insurance

Please read over our Insurance Information so that you understand the process of insurance billing and requirements for massage. This document provides you with instructions for contacting your insurance plan to determine coverage of our services.  We want you to understand your insurance benefits and our billing procedures so that you know your financial responsibility and so your time here is spent on health and wellness.

Please check with your specific insurance plan to confirm coverage of our services.

Massage - Please Read

All massage visits require a prescription prior to the first appointment. We rerquire a prescription for massage even if your plan says that you do not require a referral or prescription. We need a medical diagnosis for billing insurance and currently Massage Therapists can not diagnose for a medical condition. The prescription or referral must be faxed to our office prior to or on the date of your first visit. Most insurance plans only allow massage for a specific pain condition. Please check with your plan to see what conditions are covered.

Prescription or Referral Requirements

Your prescription needs to have massage therapist name, doctor name, diagnosis code(s), start and end dates and number of sessions. You may bring this in with you or fax to us at 206-329-2357.

Insurance and Payment Information
We want you to experience your time here with minimal effort. The following information will help you become familiar with the insurance process, including billing, copays, contracted rates and statements from Union Center For Healing and Nightingale Billing Service.

YOUR PART:
•   Confirm that your insurance covers Acupuncture or Massage and you obtain any referral or prescription needed prior to your first appointment.
•   Be sure your plan covers the conditions for which you would like to be seen; most plans only cover certain conditions.
•   Finally, keep track of the number of visits allowed on your plan.
•   Please know the contract is between you and your insurance carrier and you are fully responsible for any amount that they do not pay. Our office does not guarantee that your insurance will pay.

MASSAGE: Prescriptions are needed for massage prior to the first date of service. A prescription is required even if your insurance states you do not need one. Insurance billing requires a medical diagnosis and massage therapists can not diagnose. Insurance will not cover visits without a prescription.
The following information is required on the prescription:
Start and end dates, diagnoses, number of visits, provider or clinic name, and your provider’s name. Please note that most plans only cover specific pain related conditions. Find out what is covered under your plan to be sure the condition you want treated is covered.

COPAYS: : If you have a copay (indicated on your card) you can pay it at the time of your appointment or every 2-4 appointments. We accept Cash, Check and Credit Cards. Any copays not collected at the time of service will be billed.

BILLING: We use a third party biller, Nightingale Billing, to process claims and payments.   Claims are sent to your insurance company every week and are usually processed by the insurance company within 9-30 days. Since we are contracted with your plan, we agree to be paid the contracted rate, which is a set amount. Your financial obligation is the copay, or co-insurance (a percentage), or deductible. If you are paying towards your deductible, you are only responsible for the contracted rate. Codes billed: There are only a few codes that we use for massage. These include 99125, 99212, 97124 or 97140. Typically you will only see 1-2 codes billed depending on the type of visit: First appointment, new condition, etc.

STATEMENTS: Statements are mailed monthly every 4 weeks for balances due. These balances include deductibles, copays, co-insurances, or any balances unpaid by the insurance. The statements will only include balances due once the insurance has been processed.

PAYMENTS:You may pay your balance or copays at the center, through our website, by mail,  or by calling our biller Nightingale.

RECEIPTS: Nightingale can provide you with a summary of your visits for your FSA plan.
